Healthy Heart Plus Disclaimer and Terms and Conditions:
1. ACKNOWLEDGE, agree, and fully understand the wellness and/or aesthetic therapies provided by Healthy Heart Plus. I have answered the health-related questions honestly and completely. Though licensed and certified in her specific service being provided at Healthy Heart Plus, I understand that my Healthy Heart Plus consultant/therapist is not recognized in the state/province as a medical physician and cannot diagnose or prescribe any medications, treatments or services. I understand that I am here to learn about nutrition and better health practices and that I will be offered information about food supplements and herbs as a guide to general health and this is considered a personal ministry and counseling. The services performed by Alice Freeman, Healthy Heart Plus, its Board, therapists, employees or designees are at all times restricted to consultation on the subject of nutritional matters intended for the maintenance of the best possible state of nutritional health and do not involve the diagnosing, treatment or prescribing of remedies for disease. I understand that Healthy Heart Plus and my therapist are not liable for any unforeseen medical issues that I may experience or complications that may arise that could be related to an undiagnosed, pre-existing medical condition prior to or after my visit. I will disclose any concerns; health-related or otherwise as well as discuss any pre-existing conditions to my therapist prior to receiving a treatment.
2. FULLY RELEASES, WAIVES DISCHARGES AND COVENANTS NOT TO SUE Healthy Heart Plus, its Board, therapists, employees or designees from any and all losses, causes of action, claims, damages or liability that Client, Client’s spouse, child(ren), guests, legally authorized representative, assigns, successors and representatives may have that relates to, arises out of or in any way connected to Client’s use of the Healthy Heart Plus products, information and/or Client’s participation in Healthy Heart Plus Therapies.
CLIENT ACKNOWLEDGES THAT S/HE HAS READ THIS RELEASE AND WAIVER OF LIABILITY, UNDERSTANDS ITS TERMS, UNDERSTANDS THAT S/HE WILL BE GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GUARANTEE BEING MADE. ALL DECISIONS RELATIVE TO MY HEALTH MUST BE MADE BY ME AND I WAIVE ANY LIABILITY ON BEHALF OF HEALTHY HEART PLUS, IT’S BOARD, THERAPIST, EMPLOYEE/VOLUNTEERS OR DESIGNEES.
Appointment Cancellation Policy Agreement:
Healthy Heart Plus is committed to providing all of our Clients with exceptional care. When a Client cancels without giving enough notice, they prevent another patient from being seen. Please call us at (804) 320-1220 by 3:00 p.m. on the day prior to your scheduled appointment to notify us of any changes or cancellations. To cancel a Monday appointment, please call our office by 2:00 p.m. on Friday. If prior notification is not given, you will be charged either a percentage or full price for the missed appointment.
No-shows
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged the NON-REFUNDABLE deposit for their first “missed” appointment. If a person continually chooses not to show up for appointments, without calling 24 hours in advance, they can be charged for the entire missed appointment or blocked for an undisclosed amount of time. Fees for missed appointments need to be paid before the next appointment can be booked.
For Wellness packages and two or more guests coming together, we require a 48-hour cancellation notice. All therapies require a NON-REFUNDABLE deposit at time of booking. We do not provide refunds, exchanges, credits, transfers to other clients or reschedule (must reschedule within 30 days of the last appointment) for same day cancellations, missed appointments (No Shows), and/or out of the 30-day time frame.
Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending on how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for payment of the “full” session. Out of respect and consideration for your therapist and other Clients, please plan accordingly and be on time.
Cancellation will only be accepted via phone.
Cancellation via e-mail is not accepted. You must reschedule appointments and render services within 30 days of canceling V-steam, colonic, ear candling, ion cleanse, and/or 30-day detox consultations. Failure to do so will result in no refund/no exchange/non-transferable policy/no rendering of services.
